Metric Importers of North America (MINA)
There are 2 versions:
Version 1 is the English Language Version
English is the official language.

Version 2 is the English/German Language Version
While English is the official language other languages may be incorporated into the form. The example here is German, but other languages may used.

Also available for download is a zip file with both versions. They are in Word 97, Word 6.0, Text, and Works 4.0 formats. Some of the formats lose the rules and borders, but the text is there for you to cut and paste. The file name is conform.zip.

You may also add additional information.
